We are so excited that you are planning to attend the 2009 Hazon Food Conference. In order to make registration as smooth as possible, every individual attending must fill out a separate registration form and you will pay once at the end. If you plan to attend with a spouse, partner, children, or best friend that you would like to room with, please indicate this on the registration form in the space provided. Asilomar is a tranquil ocean front retreat cradled by forests and white sand beaches. Lodging at Asilomar is peaceful and comfortable with rooms located in buildings nestled along the shoreline forests and dunes. All rooms have private baths, towels and linens. Waiver and Release

By clicking on "I Agree", you agree, warrant and covenant as follows:

A.  I have reviewed the plans for the Food Conference (the “Program”) and recognize that there are risks to the person and property of participants who travel to and/or participate in the Program (“Participants”).  I understand that Hazon, Inc. (the “Company”) could not afford to conduct the Program, if it was required to bear the financial responsibility for those risks.  Therefore, in order to induce the Company to make the Program available to me as a Participant, I agree to bear the risk of loss arising from injury to myself and my property in connection with the Program.

B.  Accordingly, in consideration of my inclusion in the Program, I hereby voluntarily release and forever discharge the Company, and its agents, employees, officers, shareholders, members, managers, and directors, from any and all liabilities, claims, debts, demands, suits, actions, causes of action, or rights of action, which I or my heirs, assigns, legal or personal representatives, or estate can, shall, or may ever have, now or in the future, for, by reason of, or based upon any injury, death, illness, disease, or damage to or loss or theft of property arising out of, directly or indirectly, or in any way connected with or related to the Program, whether or not caused by any negligent acts or omissions of Company.

C.  Also in consideration of my inclusion in the Program, I hereby, on behalf of myself and my heirs, assigns, legal and personal representatives, and estate, further agree, promise, and covenant to indemnify and save and hold harmless the Company, and its agents, employees, officers, shareholders, members, managers, and directors, for and from any liabilities, losses, damages, costs, or expenses resulting directly or indirectly from any injury, death, illness, disease, or damage to or loss or theft of property arising out of, directly or indirectly, or in any way connected with or related to, my participation in the Program.

D. In consideration of Hazon's agreement to permit me to participate in the conference, I hereby grant to Hazon, and their respective designees, agents, representatives, successors, assignees and licensees the worldwide and perpetual right and permission to use my name, city and state of residence, photograph or other likeness, and any statement I may make during the conference (in edited or unedited form), and any written documents or photographs I submit to Hazon, in any and all marketing and promotional materials (or to refrain from doing so in its discretion) in any manner or media whatsoever for purposes of marketing, promoting or publicizing Hazon.  I understand and agree that I shall have no right of approval, no claim to any compensation, and no claim arising out of the use in any form of my name, city and state of residence, photograph or likeness.

 

Entire Agreement I understand that this is the entire agreement between myself and the Company related to this subject matter and that it cannot be modified or changed in any way by any representations or statements by any employee or agent of Company or by me, except in writing signed by both parties. I understand and agree that this Acceptance of Risk and Release shall be governed by the laws of the State of New York and that if any court should find any part of this document to be invalid or unenforceable, the rest of this document shall, notwithstanding, remain in full force and effect. I HAVE CAREFULLY READ THIS DOCUMENT AND FULLY UNDERSTAND ITS CONTENTS. I AM AWARE THAT THIS IS A FULL ASSUMPTION AND ACCEPTANCE OF RISK AND A RELEASE OF LIABILITY AND HAVE SIGNED IT OF MY OWN FREE WILL.
